It is an insurance company's worst nightmare. One of your adjusters opens his/her daily mail to find a default order was entered against the insured 45 days ago in a matter that inadvertently slipped through the cracks. Or, one of your defense counsel calls, explains that he lost track of a file, and that a default was entered against the insured more than two months ago. Your first thought "how did this happen" is quickly replaced by "what do I do now?"
